If you open these forms in Adobe Reader you will be able to fill them in on your computer and save them for printing or emailing.
In order for your application to be processed, we must receive all of the following:
- Completed Application form. On-line or via the PDF documents
- Completed Tuition form
- Completed Health information form
- Completed Emergency information form
- Evaluation form and school transcripts, completed and sent directly by school principal
- A copy of both sides of your medical insurance card
- A recent photograph. May be attached to on-line application or emailed
- $50 application fee, non-refundable (Pay on-line using payments menu)
